I am aware of and agree to the following requirements for all custom portrait sessions with DM. *
Retainer fee of $250 ($267.50 including tax) is required to reserve a session. A $50 credit will be applied to your final print order. I understand that I will forfeit my print credit if I do not place my print order within 30 days of my live gallery. If I need to reschedule I can do that once. If I cancel a second time I understand that I forfeit my retainer fee. Additional travel fees will be assessed for requested locations 20+ miles away from Dawn McCarthy Photography 32068. I understand that my online gallery will be available for ordering for 14 days. A re-host fee of $25 is required for an addtional 7 days. There is a $45.00 fee for all returned checks. I understand that the creative session retainer fee does not include prints. This fee covers photographers time, talent and equipment only. I understand that there is a minimum portrait purchase of $100.00 required to order prints. I will promptly send a check for the retainer fee to Dawn McCarthy Photography 4137 Pinto Road Middleburg, FL 32068 I understand that it takes up to three weeks for my proof gallery to go live.
By typing my name below I am authorizing my digital signature in agreeance with all of the above stated policies and the policies outlined in the DM client lounge. As well as the model release which simply states that I hereby consent, without further consideration that all photographs taken of myself and/or my children and family at this session by Dawn McCarthy Photography may be used and reproduced in printed and online publications for the purpose of marketing, promotions, and advertising by Dawn McCarthy Photography. *
Copyright Agreement: I understand that all images from my custom portrait session are the property of DM and I agree not to copy, scan or reproduce any images without written consent from DM. This includes all Blog, web, Facebook and MySpace images. DM is happy to share watermarked images on these websites and you are welcome to "tag" yourself for display on your own page but you may not "accidentally" right click and save any of these web images for print. *
